Interventions comments - increase characters limit

It would be useful, to improve records and visibility with parents mainly, to have the ability to enter longer supporting comments when assigning an intervention in Epraise.

Staff ability to 'Report an incident'

Allow staff to report incidents such as bullying and be able to triage/track the progress. Staff cannot currently do this even if masquerading as a student.
